When Is It Time for Full-Arch Dental Rehabilitation?

PrevAll-on-4 vs All-on-6: Which Is Right for You?
Fixed Teeth in One Day: Temporary vs Permanent BridgesNext

When teeth have been repaired one by one for years, yet pain, looseness, broken crowns or difficulty chewing keep returning, the problem is no longer just one tooth. It is time to assess the entire dental arch – the bone, gums, bite, function and appearance – and develop one comprehensive treatment plan.

Full dental rehabilitation, or full-arch rehabilitation, does not automatically mean extracting all your teeth, nor is the same solution suitable for everyone. The aim is straightforward: preserve teeth with a good, predictable prognosis, eliminate sources of chronic infection and restore a stable bite with a solution the patient can use and maintain comfortably.

At DentaS in Viškovo, just outside Rijeka, the decision to undertake extensive rehabilitation is made only after an examination, radiographic analysis and an assessment of the relationship between the remaining teeth, bone, soft tissues and future prosthetic restoration. The number of implants and the name of the treatment method come after the diagnosis, not before it.

What Does Full-Arch Dental Rehabilitation Actually Mean?

It means restoring all the teeth in one or both jaws as a functioning whole. The plan may include treating and preserving some natural teeth, periodontal treatment, extracting teeth with a poor prognosis, bone augmentation, implant placement, a temporary fixed restoration and the final prosthetic restoration.

For a completely toothless jaw, or when the remaining teeth can no longer reliably support a prosthetic restoration, one option is a fixed bridge supported by four, six or more appropriately positioned implants. However, the number of implants is not a marketing package. It is determined by the anatomy, quality and available volume of bone, the bite, chewing forces, the opposing jaw and the type of final restoration.

If you would like to understand the differences between these approaches, read our article: All-on-4 or All-on-6 – how is the number of implants chosen?

When Is It Still Worth Saving Individual Teeth?

A natural tooth is worth preserving when inflammation can be controlled, enough healthy tooth structure remains for a reliable restoration, periodontal support is stable and the tooth can function as part of a long-term plan. An implant is not a “better natural tooth”; it is a replacement when the natural tooth no longer has a predictable prognosis.

That is why a sound treatment plan is not based on the question “Can we somehow repair this tooth again?”, but rather “How likely is this tooth, and the entire reconstruction, to remain stable several years from now?” Sometimes it makes sense to retain strategically important teeth. At other times, a succession of costly, partial repairs simply delays the inevitable and consumes bone, time and money.

Five Signs That It May Be Time to Consider a Full-Arch Treatment Plan

  • Most teeth have a poor or uncertain prognosis. There may be deep decay, fractures, failed root canal treatments or too little healthy tooth structure to support reliable crowns.
  • Advanced periodontitis affects the entire dental arch. Loose teeth, bone loss, bleeding and recurrent inflammation may make stable prosthetic support impossible.
  • Restorations need constant repairs, yet the bite remains unstable. Broken bridges, loose crowns, pain when chewing or a loss of bite height point to a problem affecting the entire system.
  • Many teeth are missing, or a removable denture no longer provides adequate function. Instability, sore spots, a denture that moves and avoiding harder foods can significantly reduce quality of life.
  • Infections and pain keep recurring in different areas. A series of emergency procedures is not a treatment plan. When the problem moves from tooth to tooth, the entire jaw needs to be assessed.

None of these signs alone is a reason to extract all the teeth. Each tooth is assessed individually, but as part of a single plan for the entire dental arch.

How Do We Decide Which Teeth to Keep and Which to Replace?

  1. Clinical and periodontal examination. We assess tooth decay, fractures, tooth mobility, pocket depths, gum health, infections and the quality of existing restorations.
  2. Radiographic and 3D analysis. A panoramic X-ray and, when indicated, CBCT imaging reveal the available bone, the position of the sinuses and nerves, hidden infections and the anatomy needed for safe implant planning.
  3. Bite and functional assessment. We examine tooth wear, bite height, jaw position, parafunctional habits such as bruxism and the forces that will act on the future restoration.
  4. Assessment of appearance, speech and lip support. Tooth length and position are not planned solely from a smile photograph. Facial features, the smile line, speech and the amount of lost tissue all matter.
  5. Assessment of general health and habits. Smoking, diabetes management, medication, oral hygiene and willingness to attend regular check-ups influence the risks and the choice of treatment.

What Treatment Options Can Be Part of the Plan?

  • Preserving strategically important teeth and using fixed prosthetics. When some teeth have a good prognosis, periodontal treatment, crowns, bridges and individual implants may be combined.
  • Fixed implant-supported full-arch restorations. In cases of complete tooth loss or a failing dentition, the entire dental arch may be replaced with a fixed bridge supported by an individually selected number of implants.
  • Removable restorations stabilised by implants. For some patients, an implant-retained denture may be a more sensible option biologically, in terms of hygiene or financially than a fixed restoration.
  • Staged treatment involving bone or soft-tissue regeneration. Where conditions are not yet suitable for implants, the plan may include bone augmentation, a sinus lift or periodontal preparation before the final prosthetic restoration.

Do Fixed Implant-Supported Teeth Always Mean “Teeth in a Day”?

No. A temporary fixed restoration can be fitted immediately or very soon after implant placement only when the surgical and prosthetic requirements are met, particularly adequate primary implant stability and favourable force distribution. If these conditions are not met, delayed loading is the safer option. Biology is not impressed by advertising.

Even when a patient receives fixed teeth on the same day, this is a temporary restoration designed for the healing phase. The final restoration is made after the tissues have stabilised and osseointegration has taken place, with a fresh assessment of the bite, appearance and access for cleaning.

What Does Treatment Involve, Step by Step?

  • Diagnosis and a written treatment plan. We define the goals, possible solutions, treatment stages, approximate duration and costs.
  • Preparing the mouth. Active infections are treated, teeth with no viable prognosis are removed, and regenerative procedures are carried out where necessary.
  • Implant placement. The procedure is performed under local anaesthesia. At DentaS, conscious intravenous sedation is also available for patients with significant dental anxiety when medically appropriate.
  • Temporary prosthetic phase. If immediate loading is indicated, a fixed temporary bridge is made to protect the healing phase and allow the function and appearance to be assessed.
  • Healing and follow-up appointments. Soft-tissue health, implant stability, hygiene, the bite and the performance of the temporary restoration are monitored.
  • Final full-arch restoration. Once biological stability has been achieved, a precise definitive restoration is made, such as a zirconia bridge on a titanium framework, or another solution selected for the clinical situation.
  • Long-term maintenance. Professional cleaning, bite checks, X-ray monitoring when indicated and timely repairs help preserve the implants and prosthetic restoration.

Why Are Digital Planning and Temporary Restorations Important?

In extensive rehabilitation, every millimetre affects the smile, speech, hygiene and the load placed on the implants. Digital impressions, photographs, bite analysis and virtual planning allow the implants and future teeth to be planned as a single system. At DentaS, we use the 3Shape TRIOS 6 for digital scanning, helping ensure precise communication between the practice and dental laboratory.

Read more about digital scanning in our article: 3Shape TRIOS 6 – next-generation digital dentistry.

How Much Does Full-Arch Dental Rehabilitation Cost?

A single universal price would not be an honest answer. The cost depends on the number and system of implants, extractions, the need for bone augmentation, sedation, the temporary restoration, the material and structure of the final restoration, and whether one or both jaws are being rehabilitated.

That is why a final quotation is prepared only after diagnosis and a decision on the treatment approach. At DentaS, the initial examination, quotation and treatment plan are listed as free of charge, allowing patients to understand the stages and costs before making a decision, without guesswork.

View the current DentaS treatment price list or send us an existing dental scan through the contact form to help us prepare for your initial consultation.

Successful Rehabilitation Does Not End When the Teeth Are Fitted

Implants cannot develop tooth decay, but the tissues around them can become inflamed. Prosthetic screws, veneering materials and biting surfaces are also subjected to constant forces. For that reason, daily cleaning beneath the bridge, individually selected hygiene aids, regular professional check-ups and occasional bite adjustments are an integral part of treatment, not an optional extra.

A patient who understands how to maintain their restoration and attends follow-up appointments has a much better starting point for long-term stability than someone who has been promised that implants will last “forever, without any worries”. Such promises sound reassuring only until the first repair bill arrives.

When Is the Right Time to Book an Examination?

If you have several loose or broken teeth, recurring infections, restorations that keep breaking, difficulty chewing or a removable denture you can no longer tolerate, it makes sense to arrange an examination before embarking on another series of individual repairs. The purpose is not to persuade you to have teeth extracted, but to establish whether there is a more predictable plan for the entire jaw.

DentaS Dental & Aesthetic Boutique is located in Viškovo, just a few minutes from Rijeka, with free parking. At the initial examination, we assess your condition, explain the realistic options and prepare an individual treatment plan.

Frequently Asked Questions

  • Does full-arch rehabilitation mean every remaining tooth must be extracted?

No. Teeth with a good, predictable prognosis are retained if they can form part of a stable plan. Extraction is justified when a tooth cannot be reliably restored or when it compromises the entire reconstruction.

  • Is All-on-4 the same as full-arch rehabilitation?

All-on-4 is one possible approach to a fixed full-arch restoration. Full-arch rehabilitation is a broader term and may include All-on-4, All-on-6, a larger number of implants, preservation of some natural teeth, bone regeneration or a removable restoration stabilised by implants.

  • Can I get fixed teeth on the same day?

This is possible only when the requirements for immediate loading are met, including good primary implant stability and favourable force distribution. The decision is made during planning and confirmed after the implants have been placed.

  • Is full-arch rehabilitation possible if I do not have enough bone?

There are often additional options, but they depend on the location and extent of bone loss. The plan may involve different implant positions, bone augmentation or a sinus lift. An assessment is made following a clinical examination and 3D imaging.

  • How long does full-arch rehabilitation take?

The duration depends on whether extractions or bone regeneration are needed, whether immediate loading is possible and the time required for osseointegration. Patients receive an individual schedule of treatment stages after their examination.

  • How do I care for a fixed implant-supported bridge?

The area beneath the bridge must be cleaned daily using aids suited to its design. Professional check-ups include examination of the surrounding tissues, hygiene, bite, screws and prosthetic materials.

Prije dvije godine sam odlučio pregledati zube. Dočekala me je dr. Prepustio sam svoje zube uvijek srdačnoj i uvijek nasmijanoj Anabel 😀i njenoj asistentici Romini.
